Client Acces (Private)

Required forms:

Each client completes a New Client Form assessment

Each client (or the couple signing jointly) completes an Office agreement

PLEASE ASK JEFF TO SEND YOU THE INTAKEQ or email a Word or PDF version of documents.

Payment types accepted: Cash, Credit Cards, PayPal and Venmo. No checks.

PayPal instructions:

1) Click the “Buy Now” button or send payment to the email: Jeff@AliveCounseling.com
Note: Jeff Wilfong, MFT should be in upper left corner
2) “Price per item” box: Enter the fee of the service
3) Press continue

4) Login to your PayPal account or edit your payment info if you wish to not use an account.

5) Proceed until the Confirmation page.
Note: you should receive an e-mail confirmation if you succeed.
