What is Dialectical Behavioral Therapy (DBT)?

Dialectical Behavior Therapy (DBT) is a cognitive behavioral program designed and throughly researched by Marsha Linehan, Ph.D. that is used to treat individuals diagnosed with borderline personality disorder (BPD). It is the main research-based treatment program for BPD.

“Dialectical” means an integration of opposites,such as how people balance 1) Acceptance and 2) Change. It is difficult to change or accept, however, when you skillfully utilize dialectics through DBT skills training, you can move with greater fluidity and ease through life.

Various forms of DBT:

The complete form of DBT consists of 1) Skills Training groups, 2) Individual Counseling, 3) Phone coaching, and 4) Consultation teams.

Individual counseling: Therapy is focused on applying DBT skills, understanding borderline personality disorder (BPD), enhancing motivation to meet goals in treatment and life, and helping assist clients to manage difficult stresses and relationships. Most clients meet weekly for a 50-minute appointment with Jeff. It is strongly advised not to come less frequently than this. Ask about discount packages.

Phone coaching: Jeff provides scheduled phone counseling, skype/facetime counseling, and email support (all billable) for those who need coaching between sessions or when difficult situations arise. Please note that Jeff may have to contact law enforcement to come to your residence (safety check) for certain grave situations.

Consultation teams: Jeff regularly brings up cases (without any identifying information) with peers to enhance his skills, obtain alternate perspectives, and to manage complex situations.

Psychiatry: Often, BPD can manifest in severe symptoms and unstable behavioral patterns. Jeff will require clients to get a psychiatric evaluation and to consider medication if symptoms are moderate to severe. Medication can be a huge help and many different options exist! It is not required to take medication by working with Jeff, however, he may have to refer you to another therapist if you consistently show that you cannot take care of yourself in outpatient therapy.

Main skill categories

DBT includes four sets of behavioral skills:

Mindfulness: Learn the practice of being non-judgmentally and fully aware of the present-moment. Jeff has over 15 years of intensive mindfulness and meditation practice to lend his deep expertise here.

Distress Tolerance: Learn to how to experience the pain of difficult situations, without trying to change, deny, or avoid it.

Interpersonal Effectiveness: Learn how to be effectively assertive, maintain self-respect and improve your relationships.

Emotion Regulation: Learn how to adjust your perceptions of emotions so that they can shift, move, and transform into something balanced. Find peace with any emotional experience.

Jeff will focus on any life-threatening behaviors first, then will help assist clients with therapy-interfering behaviors, quality of life behaviors (relationships, job, financial, or other problems), and then help with skills training.

Homework and books:

It is required that individuals purchase one to two books on DBT or borderline personality disorder and practice the skills between sessions. Skills training groups, phone coaching, role-plays, and other methods will be used so that individuals can be successful in treatment.
